I've only told people I'm close to in my family. I haven't told none of my friends. How do you know when you're ready to disclose to others?

No... francamente.
For years I didn’t tell a single person, was probably 3 years before I told my mother, brothers and sisters . Now if I cut myself at work, I kinda freak a little and tell the boss, “universal precautions “ once I just came out and said “I’m HIV positive” and continued with the conversation. It’s not something I tell someone when meeting “ Hi, I’m William, I’m hiv positive, pass the salt please “ no….frankly
My dear….its none of his/her business unless we having sex, your doing my laundry and soiled clothing or cleaning up body fluids….ain't none of your business….this skeleton stays in the closet. Be strong, be

A Reminder for You
"Ready" doesn't mean "not scared." You will likely still feel a flutter of nerves right before you say it. That’s normal. The goal is to reach a point where your desire for authenticity is bigger than your fear.
Important Note: If you never feel ready to tell your friends, that is perfectly okay too. Your medical history is your private property. You can live a full, beautiful life only sharing this with your family and your medical team.
4. Setting the Terms
When you do decide to tell them, you are in charge of the narrative.
Be Matter-of-Fact: If you treat it like a tragedy, they will respond with pity. If you treat it like a manageable health detail, they will respond with support.
Set the Privacy Boundary immediately: "I'm telling you this because I trust you, but I need you to keep this between us. I’m not ready for everyone to know yet."
3. Choosing the Right Friend
Not all friends are "disclosure-worthy." Use these three filters:
The "Vault" Filter: Do they keep other people's secrets? If they are a known gossip, they are not the person for this conversation.
The "Consistency" Filter: Have they stayed by your side during other hard times (breakups, job losses, etc.)?
The "Benefit" Filter: Ask yourself: "How does telling them help ME?" Disclosure should be for your benefit—to get support, to feel closer, or to stop hiding—not because you feel you "owe" them the information.
