Para que quede claro, las dos drogas que me mantienen indetectable lo hacen, ya que eran agentes completamente nuevos cuando comencé a tomarlas. Entonces, aunque en 2003 tenía VIH más o menos resistente a los medicamentos, todo lo que necesitaba para recuperar el control viral era dos agentes nuevos que nunca antes se habían usado. Afortunadamente, sólo tuve una mutación en el Darunaivir, aunque nunca antes había usado este medicamento. Para superar esa mutación, se requería, y aún se requiere, una dosis dos veces al día para evitar que se desarrollen más mutaciones y permanecer indetectables. También tomo Tenofivir, así como Darunivir/Ritonivir/Raltegravir, aunque Tenofivir sólo hace aproximadamente el 5% del trabajo. A mi médico le gusta usar tenofivir, ya que también protege contra la hepatitis B. Es un poco demasiado cauteloso, pero a mí me parece bien. Digo que esta es mi octava y última combinación de medicamentos contra el VIH, porque no puedo tolerar los efectos secundarios de los medicamentos que se usan comúnmente una vez al día, como Biktarvy.

Just to be clear , the two drugs that are keeping me undetectable, do so, as they were completely new agents, when I first started them. So even though I had more or less total drug resistant HIV in 2003, two brand new agents that had never been used before, was all that was needed to regain viralogical control. Fortunately I had only one mutatation to the Darunaivir, even though Id never used this drug before. To overcome that mutation, twice daily dosing was, and still is required to prevent further mutations developing and to remain undetectable. I also take Tenofivir, as well as Darunivir/Ritonivir/Raltegravir even though the Tenofivir is only doing about 5% of the work. My Doctor likes to use Tenofivir, as it also protects against Hepatitis B. Hes a bit a bit over cautious, but thats fine with me. I say this is my 8th and final HIV drug combination, because I cant tolerate the side effects of the commonly used once daily drugs such as Biktarvy.

Well I'm on my 8th and final combination treatment . This is due complete resistance and cross drug resistance to the insufficient treatments available in the 1990s. I have no more treatment options, than the treatment I'm currently taking. This is because I cant tolerate the new once daily treatents such as Biktarvy and Doloutegravir. As my current backbone drugs are extremely potent, darunavir and raltegravir twice daily, there's no possibility of treatment failure. Brad you have nothing to worry about ,since it sounds like your on initial treatment or your first ever combination. Merry Christmas.

@A myHIVteam Member the difference between the 1st line, 2nd line, and 3rd line etc treatments is this. When you first get diagnosed, that is 1st line treatment. If for some reason you body rejects the drugs you're on, it will reject every drug in that particular group. So then the second line treatment comes up. if your body rejects those drugs, it will reject every medication in that particular group. Then you move on to the 3rd line group.
Current World Health Organization recommends standard first-line antiretroviral therapy for adults and adolescents consisting of two nucleoside reverse-transcriptase inhibitors (NRTIs) plus a nonnucleoside reverse transcriptase inhibitor or an integrase inhibitor.
Second-line antiretroviral therapy in adults consist of two NRTIs plus a ritonavir-boosted protease inhibitor.
Third-line regimens should include new drugs likely to have anti-HIV activity, such as integrase inhibitors and second-generation NNRTIs and PIs. Patients on a failing second-line regimen with no new ARV options should continue with a tolerated regimen.

@Un miembro de myHIVteam Sospecho que cuando comience mi primer ARV estaré a una distancia decente del diagnóstico inicial. Pero cuando eso suceda, será mi tratamiento de primera línea. No estoy muy seguro de por qué se usa la palabra "línea". Primer tratamiento ARV o primer tratamiento contra el VIH podría ser un nombre más exacto.

@A myHIVteam Member I suspect when I start my first ARV I'll be a decent distance from initial diagnosis. But whenever that happens, it will be my First Line treatment. I'm not really certain why the word "line" is used. First ARV or First HIV treatment might be a more accurate name.
